LIABILITY RELEASE
YEDA Liability Waiver, General Release, and Terms and Conditions of Use

Youth Education and Development Academy dba YEDA (hereinafter referred to as "YEDA") is committed to ensuring the health and safety of our students, parents and staff. We ask that all parties involved carefully and thoroughly review the following information to ensure compliance with our standards.

General Release of Liability
I agree that by booking and paying for programs, events, classes, and services, that each participant I register has my consent to participate in all activities offered. I acknowledge that there are certain risks of personal injury associated with physical activity and I agree to assume all risk of any and all injury, damage or loss of any kind that my child(ren) may sustain as a result of participation in any and all programs, events and sessions. I release and agree to indemnify and hold harmless YEDA and its affiliates, subsidiaries, members, employees, officers, instructors, partners, aides, and/or agents (hereinafter the “Released Parties”) and any county, town, school district, or venue in which the program is being held, from all claims or liability for loss, damages and/or injuries, to any participant(s) and/or any third parties person or property; caused by and resulting from any causes whatsoever.

Minor Photo Release
By agreeing to have my minor child participate in YEDA programs, event, classes or services, I grant explicit permission to YEDA and its representatives to take and use photographs of my child, participating in YEDA programs, events, classes, and services for any and all legal purposes including but not limited to publicity, copyright purposes, website content, social media content, marketing, advertising, illustrations and program details. Furthermore, I understand that no royalties, fees or compensation shall be paid to me or my minor for such photo use. I do not agree for my minor child participant photo to be used for unlawful purposes.

Covid
The coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. The state of medical knowledge is evolving, but the virus is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and possibly in the air. People reportedly can be infected and show no symptoms and therefore spread the disease. There is no known definitive treatment or cure for COVID-19. Evidence has shown that COVID-19 can cause serious and potentially life-threatening illness and even death.

The Youth Education and Development Academy (YEDA) including each of their affiliates, subsidiaries, members, employees, officers, instructors, aides, and/or agents (the “Released Parties”) cannot prevent you from becoming exposed to, contracting, or spreading COVID-19 while participating in their programs, services or premises. It is not possible to fully protect against the presence of the disease. Therefore, if you choose to participate, you or your child may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

ASSUMPTION OF RISK
I have read and understand the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself, and for my family members or others who may be exposed, in order to participate in any activities sponsored by YEDA, Courtology, Flofiyah LLC and any other business in connection to YEDA. These services are of such value to me that I accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to participate.

WAIVER OF LAWSUIT/LIABILITY
I hereby forever release and waive my right to bring suit against the Released Parties in connection with exposure, infection, and/or spread of COVID-19 related to my participation in the Course. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence, and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

OTHER TERMS
I fully understand and agree that (a) this Release is intended to be as broad and inclusive as permitted by the laws of the State of Illinois; (b) if any portion of this Agreement is for any reason held invalid or legally unenforceable, then the balance shall, notwithstanding, continue in full force and legal effect; and (c) I have had the opportunity to ask any questions about this Agreement and I fully understand its terms and meaning.
I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS WAIVER AND GENERAL RELEASE, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE. BY CHOOSING TO BOOK AND PAY FOR ANY PROGRAM, EVENT, SERVICE OR SESSION, I EXPRESS MY DIRECT ACKNOWLEDGEMENT AND AGREEMENT TO ALL TERMS LISTED HEREIN.
